Abington Memorial Hospital | The Power To Heal

               

Throughout your stay at Abington Hospital, your healthcare team, including physicians, nurses, case managers and others, will collaborate to assist you and your family with discharge planning needs.

As your discharge date approaches, the team will work with you to ensure that the best discharge plan has been developed to meet your individual needs. On the day of discharge, every effort will be made so that you will be able to leave the hospital by 11 am.

If there is anything we can do to make your stay at Abington Hospital more satisfying, or if there is any way we can assist you in your discharge, please let us know.

To facilitate the discharge process, your Abington Hospital care team will:

  • Provide education regarding your medical condition and care needs.
  • Provide information about community resources and options for the type of care that you will require after your hospitalization.
  • Coordinate with you and your family to determine the optimal plan to meet your continued needs after discharge.
  • Coordinate your discharge needs with community agencies and make referrals to such resources as: the visiting nurse, nursing facilities, equipment companies and transportation services.
  • Provide detailed information about services that will be providing care to you after discharge, including a written copy of your discharge plan and instructions.
  • If you wish, you may arrange to have any medications that have been ordered by your physician, prepared in the hospital pharmacy and ready for you to take home.

As your discharge day approaches, here are some reminders:

  • Be sure that you understand all discharge instructions and arrangements for aftercare. If there is anything you are unclear about, ask questions of your doctor, nurse, or case manager.
  • If you are going directly home from the hospital, please make your transportation arrangements ahead of time to allow your discharge to take place prior to 11am.
  • To insure that you take all of your personal belongings with you, ask a family member or friend to begin to take these items home on the day prior to discharge. If you would like any assistance in packing your belongings, staff is always available to assist you!
