1)                 Log on to www.ihi.org (you need to create a user account.  Please use first initial and last name
    as user and password as amheip).  Review the FMEA tool

2)                 Example of areas that could be utilized as subjects for FMEA include

      a.    Critical value notification (Linda Mimm),
b.      Medication reconciliation (Cathy Laurie),
c.      Attending and nurse notification of non teaching admission, (IM Chiefs),  
d.      Placement of enteric precautions for C Dif + patient (Kathy Klemick),
e.      Attending notification of mass on CXR done as outpatient (Tina Sokolowski),
f.       Code status order entry into SCM (Sharon Strohecker, Mary Ann Bolla),
g.      Central line bundle utilization (Jim Davis),
h.      AO notification of patient transfer out of critical care (Terry Reilly)

4)             Perform a mini FMEA (in 1-2 hours) utilizing the FMEA tool.  Attempt to contact a key person or persons knowledgeable about the area of interest for your FMEA to assist you with completion.  Upon completion save it to the website and export it to a Word file for upload to Performance Logic.
